ANKLE PAIN

Ankle pain and weak ankles: build the foot under them.

Ankle pain without a fresh injury often comes from a weak foot underneath and an old sprain that never fully rebuilt. When the arch drops or the feet turn out, the ankle rolls in or wobbles. Rebuild balance and foot strength with the feet pointing straight, and get any swollen or unstable ankle checked first.

Where it starts · Shoes

Shoes turned your feet out.

The ankle sits right on top of the first leak. Turned-out feet and a dropped arch roll the inner ankle down toward the floor.

Two marble statues' calves and heels from behind. Left: both heels stand straight under the calves. Right: the heels roll in, and the inside of each calf glows a dimmer red down to the heel: the inner calf is flat and asleep.

  • Problem: tight or weak

The fix: Feet forward. Point them straight. Wake the arch.

Why do my ankles feel weak or keep rolling?

Most rolled ankles heal. Far fewer get rebuilt. After a sprain, the ankle's sense of where it is, its balance, often stays dull unless you train it back. The body adapts, leans a little to protect it, and the lean stays for years.

Shoes add to it. Thick, cushioned soles mute what the foot feels, so the small muscles around the ankle react late. The fix is to give them practice: balance, foot strength and walking on real ground. Start with foot strengthening exercises.

How do the feet and the ankle work together?

The ankle sits on the foot. When the arch holds and the big toe presses down, the inner ankle bone sits high and the ankle has a stable base. When the arch drops, the inner ankle rolls toward the floor and the ankle works at an angle all day. That's the link with flat feet.

When one ankle has covered for the other for years, the weaker side may feel strange or sore as it starts carrying its share again. That's often part of rebuilding, not a setback.

How do I make my ankles stronger?

Barefoot where you can, feet pointing straight:

  • Single-leg balance: 30 seconds a side. When it's easy, close your eyes or stand on a cushion.
  • Slow calf raises with the big toe pressing down: 2 sets of 12.
  • Tibialis raises against a wall: 2 sets of 15.
  • Walking on grass, sand or uneven ground.
  • Later, small hops in place, landing quietly with the arch held.

Is my ankle stiff or unstable?

They're different problems. An unstable ankle wobbles and rolls; it needs strength and balance. A stiff ankle won't let the knee travel forward over the toes, so you lose depth in a squat or the heels lift. That needs mobility, and the ankle mobility exercises guide covers it. Many people have one stiff ankle and one unstable one.

If your knee aches along with the ankle, the two are often linked: the knee pain guide explains why.

A 60-second self-test

Feel it now

  1. 1Stand barefoot, feet hip-width apart, both pointing straight ahead.
  2. 2Press the big toe down without curling it, and lift the arch.
  3. 3Soften the knees and keep each one over the second toe.
  4. 4Close your eyes and hold it for 30 seconds.

What pulls is tight. Stretch it.

What shakes or can’t hold is weak. Strengthen it.

  • Shift onto one foot. Does the ankle wobble, or does the arch drop?
  • Notice whether one side has been carrying more of your weight.

Not sure which leak is yours?

The From the Feet Up app is a posture and movement scan for iPhone. Six standing photos, a squat, a lunge, a reach and a short walk, about five minutes. It reads five areas from the feet up and tells you what to fix first. Free for now.

Common questions

Why does my ankle hurt with no injury?

It can come from how the foot loads all day, an old sprain that never fully rebuilt, or new demands like more running. Persistent pain, swelling or pain that wakes you should be checked.

Can old ankle sprains cause problems years later?

They can. A sprain that healed without balance and strength work can leave the ankle less stable and change how you move. It's rarely too late to train it back.

How do I strengthen weak ankles?

Single-leg balance, slow calf raises, tibialis raises and time on uneven ground, done most days. Keep the arch lifted and the big toe pressing down.

Can flat feet cause ankle pain?

They can contribute. When the arch drops, the inner ankle rolls in and works at an angle. Strengthening the arch often gives the ankle a better base.

When should I see a doctor for ankle pain?

If it's swollen or bruised after an injury, you can't put weight on it, it gives way often, or the pain isn't easing after a few weeks.

Educational, not a diagnosis or treatment plan. Sharp, severe, numb or worsening pain needs a qualified professional.
